The Palliative Care Service aims to improve the quality of life for patients with a life-limiting illness. The Redcliffe Hospital Palliative Care Service operates a weekly clinic for local patients at Caboolture Hospital. The team visits wards twice a week to see patients staying in hospital. The dedicated Palliative Care Team of medical and nursing staff works closely with your healthcare team to provide you the best individualised care.
Services include: Inpatient pain and symptom assessment, management and advice, Support for advance care planning, Liaison and support to GPs and domiciliary nurses in the community, Referral to local palliative care services including inpatient units and community services , Education.
How to access this service: No external referrals are accepted for this service. You can only access this service when you are staying in hospital or if you have been referred by a hospital clinician.
